private hospital is facing serious accusations of signature forgery, prolonged medical neglect, and fraudulent billing after a former patient lodged a formal complaint, supported by documents he claims provide “prima facie evidence of forgery and fabrication.”

The patient, who was admitted between January 20 and 23, 2022, has said that he never signed the consent form for a CT scan with contrast, conducted on January 22.

“I categorically deny signing this consent form. The signature appearing on the document is not mine and has been forged,” he said in a complaint submitted on February 27, 2026.

The documents reveal that the consent form bore a sticker dated January 28—two days after the patient’s discharge and six days after the procedure. The hospital later explained that another doctor had signed the form, but the patient insists he had never met or spoken to that doctor.

In addition, the patient alleged that on January 21, he underwent an Extracorporeal Shock Wave Lithotripsy (ESWL) procedure, a non-surgical treatment for kidney stones, without proper consent.

He claimed parts of the consent documentation were left blank and that the risks associated with the procedure were not adequately explained. The patient reported complications immediately following the treatment.

On January 20, he had a CT Urogram (CTU) scan at 4.59 pm before meeting with a doctor.

“By right, I should have seen him first before undergoing the CTU,” he remarked.
