This is story of a hospital that erred in one of the most basic protocol for blood storage and management. Unfortunately, as a result of this lapse the patient lost her life.

A pregnant patient, under antenatal care of the hospital, was admitted in advanced stage of labour. The doctor performed C-section and a healthy baby girl was delivered. The patient’s condition, however, deteriorated soon thereafter.

The patient experienced severe pain in abdomen and hence, visited the hospital where doctors performed tests that reported malignant mesenteric mass. While performing the surgery, doctors discovered that the mass was very large, solid, and highly vascular extending up to liver and other vital organs.

The patient accidently slipped from stairs and injured her left arm and shoulder. Little did she know that this accident would damage her left hand permanently. She visited the hospital where a team of doctors attended her and made provisional diagnosis of fracture of left arm and collar bone with rib fracture and osteomyelitis
